To create a guide for future interns, employees, and case mangers to have a helpful and straight forward teaching guide on successfully completing case management duties. This tutorial can be helpful for filling out termination summaries, treatment plans, and 2 week closing letters.

Evidence: I created a powerpoint that will teach future employees, counselors, and interns at CPC a guide to use when completing the basic paperwork required in mental/behavioral health agencies.
Evidence Overview:
During my internship at CPC Behavioral Healthcare, I learned the importance of case management especially in the field of social work. At first, I wasn't sure how paperwork was relevant to my internship in terms of learning how to help others. Much to my dismay, after learning how to complete the termination summaries and treatment plans, I got to see that paperwork is the backbone to being able to help patients. I learned different mental health acronyms such as ODD for Oppositional Defiant Disorder and GAD for General Anxiety Disorder or M.D. for Medical Doctor. These acronyms are helpful when reading clients mental history and past medical appointment notes. While completing the paperwork, I got to learn more about different diagnoses such as schizophrenia, anxiety, depression, oppositional defiant disorder, panic disorder, substance abuse, etc. I got to learn different symptoms of these disorders just by reading paperwork such as someone who has anxiety could be restless, irritable, constantly worried, or fearful. I also got to learn how history plays a role into what makes someone who they are. It is so important to be able to see if someone has a supportive environment, learn about their childhood, and about any previous risky behavior to be able to fully commit to helping someone get the most out of their recovery and treatment. Although the main goal is to be able to effectively help clients live a successful life, the only way to do that is to read and take notes on patients past and current mental health and being able to read the acronyms and understand the patients diagnosis is a very important skill to have.

I began my internship by learning how to complete the case management forms from my fellow co-workers by asking questions and watching them complete the essential paperwork. My co-workers were extremely helpful and knowledgable but I wanted to pass on what I learned from them and my first hand experience in order to make a guide for future employees and interns at CPC. I thought that this guide could be helpful not only to see everything that goes into successfully filling out a treatment plan but to also see what important information that people learn when filling out the paperwork such as past history and medical appointment notes. The case management notes explains what resources could be beneficial to clients and gives further insight into the next steps in a clients treatment to remain effective. Everyone has to fill out paperwork in the healthcare industry in order to give clients the right kind of recovery treatment so I hope that by creating this powerpoint that it will make the process for interns and employees a smoother process.
